5. Is CAL (Clinical attachment loss) the same thing as recession?
6. Obesity is something we should look for when diagnosing perio. How is being obese associated with perio?
7. What are the causes of localised periodontitis?

5. No
ReplyDeleteRecession is the measurement of the loss of gingiva from the CEJ to gingival margin
Whereas CAL, is the measurement from the CEJ to the base of periodontal pocket.
NB- should not take CAL from ginigval margin as this may give a false reading (due to inflammation or recession)
Localised Perio
ReplyDeleteKnown causes:
•Cracks/ splits in tooth roots
•Anatomical defects (eg the di-li groove on upper laterals)
•Retrograde periodontitis – pulpitis related to caries, leaking restorations
•Periodontal abscess
•ANUP (acute necrotising ulcerative periodontitis)
Yep..sure but there's more !!
ReplyDeleteOther causes of local perio?
ReplyDelete- puberty: hormonal imbalance
- dental appliances, such as braces, could become a bacterial niche
Adding to Renae re: false readings of inflamed gingival pockets - otherwise know as 'pseudo pockets'
ReplyDeleteAdditional causes of localised perio:
- iatrogenic factors inadequate failing restorative dentistry, root fracture due to failed endodontic treatment, body piercings (tongue rings, labia / lip etc.)
- undiagnosed caries particularly root
- root morphology - extensions, pearls, developmental grooves
Ok..puberty and hormonal imbalances do not cause localised effects as they are a systemic issue, it is more the overhangs, crowded teeth, bands etc.. that would cause the localised issues which can be exacerbated by these systemic issues.
ReplyDeleteHope that makes sense. Cath..good addition to the list.
