For Blue Group: Annabel's history

Ok guys.. lets start some thinking in this space. What other information do you need to get from Annabel in order to determine if she is affected by the same condition as Keith is?

  1. Hey guys, ive just written up a summary of how we could start off the answer to the first question. Lina and I have discussed this part in class, so i've made a bit of a summary of what we know so far! hopefully it will help to get our heads around the 'family connections'

    WHAT WE KNOW: [Annabel]
    Annabel is –
    • Keith’s first cousin
    • She has defective permanent incisors 11, 21, 22, 12 (localized)
    • Her deciduous teeth were ‘beautiful’
    • Opaque white enamel on 12,11,21,22 (localised)
    • Fine, horizontal ridges in enamel (generalised – all teeth effected)
    • Mum has perfect teeth, just like the rest of her family ( must come from dad’s side)

    WHAT WE KNOW: [Keith]
    Keith is –
    • Still acquiring a mixed dentition (missing 22 ; and has all deciduous other than 11, 21, 31, 41, 32, 42, 16, 26, 36,46,
    • All of his teeth are showing defective enamel (generalised)
    • Differences in colour of teeth present – more than one problem present?
    • Has similar deciduous teeth to permanent teeth (size and structure)
    • Keith’s teeth are not like his mother’s (rough before capped) or fathers (‘normal’ teeth)
    • Kieth’s grandfather (mum’s dad) – gaps and quite dark – similar to Keith

    LINKING ANNABEL AND KEITH:
    • Annabel has fine ridges running around her normal sized teeth / Keith’s teeth are smooth and abnormally small
    • Annabel has discolouration of her enamel (opaque white & yellow)/ Keith also has discolouration of his enamel (unsure??)
    • Annabel’s permanent anterior’s are discoloured / Kieth’s permanent anterior’s are discoloured - (still to lose posterior teeth)
    • It is not evident at this stage that Annabel’s condition has come from her mum’s side [may be from dad’s/ environment] / Keith’s condition is not like his mother’s either, however, may be linked to his mother’s dad [ may be from dad’s / environment also]
    • Annabel’s deciduous teeth were ‘beautiful’/ Keith’s deciduous teeth are defective.

    OTHER RELEVANT INFORMATION:
    • Keith is younger than Annabel


    WHAT OTHER INFORMATION DO WE NEED TO PROPERLY LINK ANNABEL’S CONDITION TO KEITH’S?
    • Does Annabel live in/ expose herself to a similar environment as Keith?
    ▬ Where she was living at the time of the eruption of her anterior teeth
    ▬ Water (fluoride exposure)
    ▬ Food
    ▬ Brushing Habits (hard soft)
    ▬ Stresses / lifestyle
    ▬ Medications
    ▬ Medical conditions
    We ask this because of Annabel and Keith’s age difference, yet similar circumstances happening at the same time.
    As Annabel’s deciduous teeth were unaffected, something must have gone wrong (maybe environmentally) at the eruption of her permanent dentition. The eruption time of Annabel’s permanent dentition may have occurred at a similar time to Keith’s deciduous eruption – where they may have both been subject to similar environmental conditions.

    • How exactly are Annabel and Keith related? Does Annabel have any direct relation to Kieth’s grandfather (whom seems to have shown similar dental characteristics to both patients.

    • Rate of change in the condition – whether it worsens at the same rate with either patients, or whether it just stays the same.

    • How has the condition changed over time (did it start off with similar colouring and thinness to Keith?)

    I thought for his question that comparisons between Keith and Annabel's teeth were important. hopefully this helps :)

    Also, i will try and get a hold of some information reguarding the importance of history taking (and some examples), so we can try and apply it to our section of this case.

  3. Joanne and I were answering the question 'What sorts of questions would you ask Annabel and her Mother?'. We came up with a few and some overlap with what Georgia and Lina wrote but here are the ones we came up with in class:
    - Did you take any antibiotics during the pregnancy?
    - Were you sick during your pregnancy with Annabel?
    - When did the discolouration occur?
    - How many times a day does Annabel brush her teeth?
    - Which toothpaste does Annabel use? (Fluoridated?)
    - What is Annabel's diet like?
    - Does Annabel snack?
    - Does Annabel eat lots of sugary or acidic foods?
    - Does Annabel brush her teeth and then go straight to bed? or Does she drink cordial/Juice before bed?
    - Has Annabel had any trauma to the face?

    We thought it would be important to check the last examinations of the other family members. We thought this because it may appear that the other members of Annabel's family have 'beautiful teeth' but they may possibly have the same condition on the posterior teeth.
